For me, it has to be Vic Fontaine (sp?). Nothing gets me to change the channel faster than a Vic heavy ep.

No one knowing what the Romulans looked like before "Balance of Terror." It just never made much sense to me that this "great foe" was never seen.
 
Anything involving the Pah-wraiths in DS9.

Also, TOS "Turnabout Intruder" - women cannot be Starfleet captains. :rolleyes: I think don't need to explain why.

And, yeah, the unseen Romulans thing from "Balance of Terror" has proved to be something of headache over the years. Thank goodness we don't have to worry about that anymore!
 
Voyager's invulnerability shield and shuttlecraft replicator, and other such impossible nonsense. They should've truly suffered from what they were going through.

Oh, and airlock Seven of Nine.
