What Star Trek have you been watching? I've been watching Trek first-run since 1969, and I can tell you that you are completely off the mark here <--- and this extends to all Berman era 90s Trek too. It didn't make much of a conscious effort as (for example) in TNG season 1, it was established that Worf grew up on a farming Colony on a planet named Galt. Then the next time it comes up in TNG season 4, Worf has suddenly been raised on Earth all along.In the past, they made far more effort at at least minimizing the discontinuities.
I know some fans like to believe their favorite version of Star Trek somehow was more consistent than all the previous versions; but that's just a load of crap. They do what they need to do to tell the story they want to tell at the time they're telling it; and no they don't always bother to check what they've established for a character in previous episodes.
The fallacy here is believing that somehow Star Trek has always been a fully internally consistent universe; aside from a few hiccups here and there. Trek has always been very inconsistent with small details, he has no problem forgetting or rewriting previous canon elements if those elements interfere with the story they're trying to tell at the time and episode is made
