I'm curious was anyone actually surprised by the ending, at least in concept?

With the episode named Terrarium?
Surprise, as in withholding information until then very end, like the scavenger ship being humans. Withholding information makes it functionally irrelevant.

ETA: Ortegas earned her humanity on her own and on her own terms. The Metrons' commentary was unnecessary for the story.

TNG did the same thing IIRC. I don't recall if it was a vaccine, but there was a medical emergency with a time limit.

Not excusing this episode doing it, just pointing out that I believe it has happened before in Trek.
TOS did it a couple of times as well, but the closest example to this SNW episode would be TOS S1 The Galileo Seven.

Two more examples of this from TOS would be TOS S2 Obsession and TOS S3 The Cloud Minders.

Meanwhile back on the Constellation--

Masada: Captain Decker, would you PLEASE stop nervously shuffling those datacards in your hand repeatedly?

Decker: I'm sorry, still nervous the Enterprise hasn't shown up with the vaccines yet.

As others mentioned, this episode probably would have worked better if the Enterprise had gone off and delivered the meds then returned for Erica. Uhura’s emotional maturity this episode was an issue.

Read between the lines. Ortegas is queer-presenting, the Gorn is a giant lizard woman. They bond together in a desperate situation and Ortegas overcomes her misconceptions. That screams queer episode.

I think you’re reading way beyond the lines to something that wasn’t there
