All of these scenarios are equally valid:

• Picard is dealing with the illness
• He used his foreknowledge to purposely prevent the illness
• His foreknowledge prevented the illness inexplicably (butterfly effect)
• The illness already wasn't going to develop. Q invented that future or showed an alternate timeline

I agree it would change his life but it wouldn’t change he got the illness.

I see no reason to assume that he is destined to get the disease. Even so, his knowledge of the possibility of the development might make him more wary.
