All of these scenarios are equally valid: Picard is dealing with the illness He used his foreknowledge to purposely prevent the illness His foreknowledge prevented the illness inexplicably (butterfly effect) The illness already wasn't going to develop. Q invented that future or showed an alternate timeline
I see no reason to assume that he is destined to get the disease. Even so, his knowledge of the possibility of the development might make him more wary.
