Umm, Eugene never claimed to be a medical doctor just that he was a doctor, he has a doctorate in sciences which could be true and not entirely part of his "lie." The lie was more he had/knew of a cure for the walker sickness.
Talking Dead the actor said the death of the doctor was strategic...which I could see to take out the actual medical doctor,so when the war happens, Negan's men will die (while the Hilltop might be able to save some rebels). Also, Eugene has those poison pills...which I think was a test from Negan. Had Eugene gave them to the wives, Negan would've said he's a traior or something, and whacked him. But Negan said they only gave good reports, despite their telling him he's a good man (which would have been a way to "expose" the potential rebel)."Hostiles and Calamities"--

I also don't believe Eugene is playing Negan. Eugene feels safest when there's an authority figure in place. Negan seems to appreciate Eugene's talents and reward Eugene with some Yar's Revenge, whereas everybody in Alexandria treats Eugene like a shitstain.
Regarding Eugene's alignment comment. Most people here seem to be missing that he was qualifying his earlier statement of "I am not good in any sense of the term" by following with the "neither lawful, chaotic, or neutral". As the common description in Dungeons and Dragons for a character's alignment would be for example: lawful good or chaotic good, or neutral good.

"Chekhov's Pill"I can see one of the protagonists taking it when they cannot take anymore (or cause more tragedy). Eugene, Carol and/or Rosita could be candidates.
If it was Dwight's strategy to get Carson killed, then it was poor, as he might end up needing medical care when forced to fight with his fellow Saviors, and Eugene is no physician.
If that letter really has turned Dwight away from Negan for good, then I'm not sure he really cares about whether there'll be medical attention for him or not. The love of his life basically just told him he was a monster who scared her so much she'd rather live alone in constant fear of zombie ambush, and it was all because of Negan.
Other than ignoring it all and staying with Negan, there are really only two responses to that: leave the saviors entirely or go back and actively obstruct/destroy Negan's group, and if Dwight was just going to leave there would've been no reason for him to come back at all.
