According to someone in the comments, there is a description of the bridge in 'The Making of Star Trek' from '68 that does describe the turbolift being directly behind the captain.
So it's possible they tested it at one point, but preferred it being over the captain's shoulder. Probably made for a better filming angle when people were exiting it.
