Actually, Joseph stays. He's not the CMO in the original series, but Joseph M'Benga appeared in two episodes of the original series still assigned to the Enterprise.

Just occurred to me that Crispin Glover sued over them making an old guy look like him in Back to the Future 2 but they replaced Jeffrey Hunter here and no one batted an eye.
And Sean Kenney is still alive and making appearances and giving interviews! Three of the Pike actors are still kicking!
