Considering he's got several EH's as crew and he talks to them like they are real, that is probably the exact response the writers were looking for.

His EMH is English. His ENH is Irish. I'm waiting to see what his Welsh holo will be.

Since so far they both look like him, it's probably just one main program that is integrated into the ship, but Rios has created individual characters to keep things stimulating.

They're separate programs.
And the EMH called itself the ECH when the command routine was run. Still was one program running behind the scenes.
Ultimately any hologram is just a costume worn by the computer generating them.

Well... WINDOWS has always had several distinct things it can do, but it's still all just WIN10 now-a-days.
