Promotion to Fleet Captain need not be witnessed on screen nor does it preclude Pike relinquishing the Enterprise at a later point in time (for anyone who requires a far stricter adherence to minutiae than I do in their entertainment). See? No problem. (Oops.)

Just like no knowing Spock's parents, brother, etc?Kirk would have to be blind, deaf and dumb to not pick up on the relationship between Pike and Spock.
