Captain Marvel (2019)

I thought we already knew she wouldn't be in the first one (like Hope Van Dyne), but I would be surprised if she's not in the second film.

Probably the simplest approach is how have Carol travel through a wormhole with a time differential, essentially acting as a portal into the "future" that is out present day.

You want something that you can end on a cliffhanger and pick up again without too much need for exposition. While it's safe to assume a lot of people who go see Infinity War 2 also saw Captain Marvel, it's not wise to assume that the majority of the audience did so. So it's best not to over-complicate it. Particularly since for any movie with such a huge cast, exposition scenes are going to be at a premium, which means some very economical storytelling.

Shortly after the events of The Incredible Hulk, General, now Secretary of State, Thaddeus Ross had a heart-attack while on the golf course.

After hours of surgery, appropriate rehabilitation and gaining certain perspectives that he previously lacked, Ross fully recovered...

...or did he?

After going on "a break", Pepper Potts suddenly and inexplicably goes back to Tony and is far more tollerant and accommodating of his nonsense than she's ever been...OR HAS SHE?! ;)

In all seriousness, if they're going to do this, it needs to be an actual Avenger for it to have any serious impact.

Yeah, I'm pretty sure they indeed already made this clear quite some time ago. Last year, at the very least.

I read a theory somewhere that Captain Marvel ends up trapped in the quantum realm and returns in a post-credit scene in Ant-Man and the Wasp, but I don't think I'd like her first appearance to be in the post-credits of some other hero's movie.

I prefer the wormhole/time dilation idea.

Oh, I quite agree, but one doesn't preclude the other. Remember in the comics, Skrulls also replaced the likes of Jarvis, Dum-Dum Dugan and even Amadeus Cho's pet coyote, Kirby.

Secretary of State and the CEO of Stark Industries would be plum positions for Skrull infiltrators.

That sounds more like pure fan invention to me. Aside from, as you say not being the best way to introduce a new character, it would be a bit redundant to have *two* separate female superheros coincidentally trapped in the same realm for several decades. I mean who else would they have hiding in there?! She-HulK? Black Cat? Madame Web!? ;)

How do you think they plan to introduce the Fantastic Four and the X-Men once they finally get the rights back? :D

^^
That lady is attached to everything.

Does anyone have the number for her agent? :D

When asked about Carol's whereabouts throughout the previous 20-odd movies, Kevin Feige replied...

So, where has she been over the course of the near 20-movie Avengers story? I spoke to Feige at the junket for Thor: Ragnarok and asked the man himself. I said, “Having a character that powerful out there might be confusing...” to which Feige interrupted: “The key is...‘out there,’” as he put his arms in the air.

Mar-Vell was trapped in the Negative Zone for a while, getting out by trading places with Rick Jones. Perhaps they could so something similar with Carol, not with Jones but some other character. Of course, it's very possible that Marvel doesn't have the rights to use the Negative Zone (a FF concept), but if they could, it would be a nice homage.

It would be cool to finally, officially*, bring Rick Jones into the MCU.

*There was a reference to Rick Jones in the first Hulk movie, as a "known associate" of Bruce Banner's, during the opening credits.

The Negative Zone would be great, but it loses much of its impact without the Fantastic Four history that defined it.

They've already introduced alternate "dimensions"/pocket universes in 'Doctor Strange', so the concept is established. They can quite easily do something very similar but just not explicitly call it "The Negative Zone", or anything specific at all really.
Personally I'm more fond of the time dilation approach since we already have the original Wasp stuck in the "Quantum Realm". Might not want to lean on that concept too hard, especially if there's an outside chance they ever get The Fantastic Four back.

Maybe at the end of Carol's last mission she had to use her powers to fly *really* fast to Chekhov's Gun the McGuffin and got almost up to light speed before being dumped out at the ass end of space. By the time she flew back to Earth a few subjective hours for her turned to be a few decades for everyone else.

See also: flew too close to a black hole. Passed through a wormhole. Used some alien hypergate thing. Something spacey at least!

Yeah, he's a good choice. He seems to be one of the newer go to actors for villain roles now, so I guess it was inevitable that he ended up in either a Marvel or DC movie.
