What exactly determines who maintains their memory of the Prime universe?
Q, apparently.

I don’t see why Q would need to go back and time to change anything, he could literally do it with the snap of his fingers.
Perhaps so they'll have a chance to undo it, solve the puzzle, and learn something along the way that gives them a second chance in the restored timeline.

I'm guessing they'll have to rush him to some 21st century hospital and get a taste of medical care for the uninsured.
They'd have much bigger problems than insurance there, given his Vulcanoid physiology.

My prediction is that the crew will restore the original timeline and when they go back, they find themselves back where they started on the Stargazer, but this time Picard will choose negotiation with the Borg Queen which lead to some type of peace treaty between the Federation and the Borg.
 
Q doesn't snap his fingers to get this done completely. He changes the timeline by giving something to remove a barrier for Adam Soong, and Soong has a statue in 2400. Humans went wild with genetics, moreso than any other race, and that's what helped them conquer the galaxy. Soongs' actions will be stopped in 2024 to set things right.

To me, Q’s intentions are pretty clear. He introduced the Borg to the Federation. As his last interaction with humanity, he’s going to ensure that they join the Federation once both sides have learned their lesson.
I really feel like Q's line about fear is the key here. Picard's fear of the Borg led to him destroying himself, his fleet, and any chance of lasting peace with the Borg despite the Queen's behavior being extremely atypical and thus he made the wrong choice and this whole thing is Q giving him a chance to fix his mistake, atone for his actions aboard the Stargazer.

I dunno. If humans embarked on massive genetic augmentation, wouldn't the butterflies mean that no one from Picard's era would exist any longer?

Yes, I know, they shouldn't anyway, but it makes it even more implausible.
 
Splendid follow up to the splendid opener.

This alternate timeline truly is awful. I like how Picard and 7 masterfully impersonate their alternate selves, I didn’t expect her to be president and it works great. I’m not sure why Picard says that Q sounds incoherent though. Only nitpicking: so La Sirena has no crew? How convenient!

Jurati is probably the worst possible person to bring along in this kind of impersonating business, but she did manage to come out on top (sorta: 7’s husband didn’t seem so convinced).

The old and weakened borg queen was interesting, I suspect her plan differs slightly from picard’s, though.

In any case, well written and we’ll executed, looking forward to the next. Another 9!

Now they know the year and place of divergence, I hope the Queen will be able to help a bit more on who to save (or kill), or it won’t bode well.
 
the watches are the white eye'd people we've seen in a couple trailers.
At least that's what one of the trailers implies based on the editing.

I do wonder about calling them "Watchers" since that was a term used in a pretty famous third season episode of TNG - where Picard & co unwittingly violate the Prime Directive and end up being worshipped as gods.

Clearly things don't work this way, but maybe the "Watchers" are an alien race (or group of them) and due to the TL change, humans discover them and react very, very badly to the discovery of intelligent alien life.

I think that would be really cool. It would give deeper meaning to Q's entire arc from Encounter at Farpoint to now and would make Q a very noble character (IMO) that basically shaped the entire galaxy for the better.
 
Adam Soong (based on the trailers at least) seems to be the lynchpin, we see Q giving him something that is shaped like a built, but with liquid inside.

Plus when we see his holostatue in the harbour in this episode, we hear Brent Spiner (so probably Adam) repeating Evil Picard's words 'A safe universe is a Human universe' or whatever the line was.

There's also a scene in a trailer of a Vulcan mind melding with a boy, wearing what looks like 21st century looking clothes, in a forest. The Queen in this episode during one of the ramblings mentions a forest.
