They'd have much bigger problems than insurance there, given his Vulcanoid physiology.I'm guessing they'll have to rush him to some 21st century hospital and get a taste of medical care for the uninsured.

Does anyone else think the "Watcher" in the past they're supposed to find is Guinan? Guinan was in the 19th century California in Time's Arrow.
I really feel like Q's line about fear is the key here. Picard's fear of the Borg led to him destroying himself, his fleet, and any chance of lasting peace with the Borg despite the Queen's behavior being extremely atypical and thus he made the wrong choice and this whole thing is Q giving him a chance to fix his mistake, atone for his actions aboard the Stargazer.To me, Q’s intentions are pretty clear. He introduced the Borg to the Federation. As his last interaction with humanity, he’s going to ensure that they join the Federation once both sides have learned their lesson.
Q doesn't snap his fingers to get this done completely. He changes the timeline by giving something to remove a barrier for Adam Soong, and Soong has a statue in 2400. Humans went wild with genetics, moreso than any other race, and that's what helped them conquer the galaxy. Soongs' actions will be stopped in 2024 to set things right.

There's also a scene in a trailer of a Vulcan mind melding with a boy, wearing what looks like 21st century looking clothes, in a forest. The Queen in this episode during one of the ramblings mentions a forest.I do wonder about calling them "Watchers" since that was a term used in a pretty famous third season episode of TNG - where Picard & co unwittingly violate the Prime Directive and end up being worshipped as gods.
Clearly things don't work this way, but maybe the "Watchers" are an alien race (or group of them) and due to the TL change, humans discover them and react very, very badly to the discovery of intelligent alien life.
Plus when we see his holostatue in the harbour in this episode, we hear Brent Spiner (so probably Adam) repeating Evil Picard's words 'A safe universe is a Human universe' or whatever the line was.
