My guess is that Ramsey did something heroic to get an early promotion. Maybe during the Dominion War.

So I got the impression that yes, you can refuse a promotion but that doing so too many times can hurt your career in Starfleet. Refusing a promotion once because a special circumstance might be ok, especially if Starfleet agrees with your reason. But if you continue to refuse a promotion when Starfleet thinks it makes no sense then you are sending the message that you don't want to advance in the organization and therefore Starfleet can't really count on you. It also says that you are selfish, putting your needs ahead of what Starfleet needs. I've never served in the military but I imagine it might be similar in the US military. They expect you to be a team player and advance in rank to serve your branch. So if you refuse it looks bad. It looks like you don't care.

When I was in retail this was also an active concern amongst management teams where promotion was less likely due to less stores to move to in the area. So the district supervisors would do temporary transfers to other stores to support maintaining efficiency rather than getting stuck in bad habits.It's worth noting that the U.S. military puposefully rotates troops around, which Starfleet does not. Basically this is done to ensure that you don't end up with crooked divisions, where years of service together causes the bind between individual servicemen to be greater than their duty to the chain of command (police forces, on the other hand, don't do this, and you can see what happens).

I can sympathize with this. When I was doing custodial work I fell in love with the school that I worked at. I wanted to become the new head custodian once my boss retired so badly I could taste it. When the time finally came, the district decided to hire from outside. Everyone close to me said that I should just be patient as other head custodian positions would arise, but I wasn't interested in any other head custodian position. I wanted MY school. I never put in for another head custodial position and, in fact, left the school district 2 years later after an 11-year stint.I always felt Riker did want to be a Captain, He just wanted to be the Enterprise Captain.
