Many times. That's why I said it seemed so obviously trying to draw the audiences attention to it.
In fact, Lorca may keep it there for that very reason. If in fact they do bring back the smooth headed human-like Klingons, and keep with Enterprise continuity, he would be aware of a Klingon's ability to easily appear human. Klingons may even have tried to pass as human before.
