Assuming you mean as I escape aboard a passing extraterrestrial space vessel?
I would take:
Four very big and glossy hardback books I have, which are:
1. About Earth, its geography, its mineral composition, with lots of detail of its mineral deposits, cave systems, river systems, mountains, valleys, tectonics, oceans, cities and other points of interest.
2. About Earth's animal and plant life, in its great variety.
3. About Earth history.
4. A detailed Atlas.
I will be able to preserve our world and its people in memory, and in alien records (the ETs can translate it all into their databases, I'm sure)
As for the other 5:
5. Samples of Terran food and drink, so they'll be able to replicate nourishment suitable for my body chemistry.
6. My Ipod, so as to preserve our music (or at least that music I like
)- I'm sure the advanced ETs will be able to copy and reproduce it.
7. The Quartz crystal I carry with me almost everywhere (long story)
8. Pen and paper, to record memories of my life and experiences as part of my "save records of humanity and Earth" idea.
9. A towel, obviously.

My guitar, a cd player, my amp, my bike, the entire wine yield of Spain from 1991 to 2009, a hydroponics bay stocked with cuttings and seeds of the the 15 best cannabis plants ever, and a lifetimes supply of Fillet Steak and vitamin pills.
