Kirk "assumed" command of Enterprise, but did not "take" command until given an order by Starfleet Command at the end of the movie. Whilst the two terms are used interchangeably by most people, the correct way is as I have just explained. Thanks!

How exactly could Kirk have graduated from the Academy as a full Lieutenant? That's the equivalent of an Army Soldier getting commissioned right out of Officer School as a full Captain.
You know, though...I think that the two of them still have very different personalities. Macet is a quieter, think-first-THEN-speak person. His gestures aren't as sweeping, as egotistical and arrogant. You can believe he got stuck having to defend a really, really shitty policy by his government, rather than that he was deliberately trying to screw everyone over. Everything about Dukat's tones and body language is different.
What all three of those first Cardassians did was provide "archetypes" for the varied Cardassian personalities we saw later on. They weren't simply 2-dimensional aliens-of-the-week that you could write off as all being the same.

Their ranks are related to being Doctors. All they had to do was go through a brief officer/military training course to become Captains. Other professionals are granted the same.I believe that in the Korean war, MASH Doctors were commissioned as full Captains so they had rank to use
I'm with Wormhole on this one, it doesn't seem to be a continuity error so much as it is a retcon. The difference between Q Who and BOBW is even addressed by Shelby and Crusher, implying that the writers willingly and knowingly made the change. When you start killing them, they definitely take notice. (by definition, it can't be a continuity error if that "error" is fully addressed and explained).
As well, Voyager also had a few instances when a boarding party could sneak by a few drones out in the open much like in Q Who? It's only when the Borg have a specific reason to be concerned with the crew that they notice and take action.
