Based on the dialogue, it looks there must be several switches between the two men called Lazarus that happen off screen as has been said.

Spock later says the phenomenon occurs "when Lazarus has his alleged confrontations." Onscreen the had only been a couple, so that suggests to me there were more.

Despite briefly seeing Leslie in the chair, this is also one of the episodes that could have had Uhura in command, but off screen.

I don't have any tattoos or piercings or whatever... I like my body too much to damage it. If I want a picture on my chest I just put on a T-shirt with one.

The last time I watched it I really enjoyed it. I saw through some of the flaws.
