I doubt I would include an option for a widowed friend to bring a +1, unless I knew s/he was actively seeing someone, actively interested in seeing someone, or had been widowed many years ago. If the person in question were widowed with in the last six months, I absolutely would omit the option for a +1, unless, again, they were seeing someone.
Certainly if I omitted it and the RSVP indicated a +1, I wouldn't blink.
The point is, at least from where I sit, that some sensitivity would be required from me, and I would have to gauge the situation accordingly.
My paternal grandfather has been dead for over 25 years. When my grandmother gets a wedding invitation, it upsets her to have the option of a +1. People who really know her simply invite her. People who know her very well address the letter to Mrs. <My Grandfather's name.>
Emily Post may allow such a thing, but kindness and compassion overrule EP in this case.
As I don't know Thena's Dad's situation, it would be impossible to speculate on whether or not an option for a +1 would or should be included.
