Voq/Ash surgery and Klingon code of honor

But from my understanding, they took a human, and implanted Voq's memories / psyche into Ash Tyler's body.

It wasn't so much of a physical surgery so much as a mind transfer to co-exist inside a human body on a sub-concious level, ergo the perfect sleeper agent.

We saw scenes of the surgery in flashbacks, they ripped Tyler apart to turn Voq into him.

Plus all the scaring that Culber noticed when doing his deep scans.

So they did a Dr. Frankenstien-esque fusing of two bodies & minds?

I thought the scars was from the torture & rape

The Klingons knew that StarFleet could figure out any surgeries for a Klingon disguised as a human, but figuring out what happened to a person mentally is far harder and not as clear.
I understood it as they surgically altered Voq to appear human; using Tyler as a template. Not sure if Tyler was also a starfleet officer who was killed by the klingons or a made-up personality.
