This is a point that bugged me for this episode. Kirk, Spock and McCoy are basically bystanders in resolving the problem. It's the freakin' Counselor who figures it out! ...And this comes back to the sense of stories being written with something of a TNG sensibility to them.

Agreed.I've been a big McKennah supporter since STC began; I certainly wasn't complaining about her having a more active role in the episode, just the relative sidelining of Kirk.

Probably. Outside of The Fugitive definite endings for series weren't common and certainly not for some years later.With just another episode, most likely, not unlike Mission: Impossible ended after seven seasons.
