Yes, "trained from birth to skip foreplay" is a very disturbing part of their culture
I'm guessing that the 'concubine guards' are trained in such that they don't require foreplay to be able to perform.
(as opposed to forcing themselves upon a recipient)

Also, they sat there on the bed waiting for Billups to come to them, they weren't apparently going after him to initiate the sex.
(beyond being ordered by the Queen to be readily available)

There are probably cultures like that. But, as several people pointed out, the "marry the princess and rule the kingdom" isn't just a trope, but was (is?) actual historical fact.

I always advise myself to not try to understand Star Trek distances. The writers hate it as much as I do.

Re: Wussian. It was in the episode "Carbon Creek" I believe. It was so the Vulcan lady who was T'Pol's ancestor could pay for some human kid's school, or something? The Vulcan gave Velcro to some human businessman.
the whole thing is dubious, though, as the episode heavily implies T’Pol made up the story.
 
I just had a thought:

Does the Hysperian law which mandates Billups becomes king once he has sex, only apply if that sex is with another Hysperian?

Meaning, if Billups sleeps with, say, one of his crewmates on the Cerritos, the law wouldn't kick in, would it?

Also, how could Billups become king if his mother is still alive?

Billups makes no indication that would make a difference. The real question is how would his mom find out about it? It could be a matter of honor that was drilled into him from childhood. He is honorbound to become King, so he couldn't live with himself if he kept it a secret.

Also, how could Billups become king if his mother is still alive?

Lots of Monarchs abdicate for their children for various reasons. This would allow the Monarch of Hysperia to retire early and enjoy their Royal Retirement. No one rules for more than 25 years or so, and it's while they are Young and Dynamic. This makes for a more exciting Ren-Faire experience for everyone else.

I think it was pretty heavily implied he had to remain a virgin and didn’t matter with whom he did the thing.
The real question is how would his mom find out about it? It could be a matter of honor that was drilled into him from childhood
I think so.
