Well in Shatner's Kirk novels Kirk is alive in the 24th Century which would explain how we could see him and Spock Prime together. i know they wouldn't use that though but maybe they just won't explain it and we are left to fill in the blanks ala X-Men Days of Future Past.
I want to point out there was a rumor about casting a young Deanna Troi, maybe she'll be with future Kirk and Spock?
I'm thinking they may move forward with a Next Gen reboot as a sequel to the NuTrek movies since there's some concern about the cast doing a fourth movie. Older Kirk and Spock could appear in Next Gen movies perhaps, although Kirk would need to be aged more, as McCoy was in Next Gen tv series.

It's McCoy; it could easily be a shorthand reference to having to assist with a particularly complex and, let's say nasty Gorn birth...I'm curious if the novels are going to deal with McCoy's line in ID about giving a C-section to a pregnant Gorn, when in the novelverse the Gorn lay eggs.
That's the thing about medical science. You never know what you'll run into. The egg didn't release with the others, no one realized the egg was left inside, it developed inside the Gorn, and a C section had to be done as the egg was beginning to hatch. All concerned were just lucky enough to have McCoy there when it was hatching and the C section had to be done.
