With transporter and synthisizer/replication tech - I think you could say it's doable.When you've done that much damage to a body to transform it, I don't really see how you could hide it to any great degree. Especially from sensors. Even if they could, it seems like they would've done the deep tissue scan or whatever it was Culber was doing immediately when they got Tyler back from the Klingons. Because it took like five seconds for him to figure it out, at that point.
That was one of those things where I don't think the writers really thought through what they were putting out there.

To be fair though, if there's an infiltrator (like Oh) this far up and with sufficient security clearance, making sure that your chess pieces aren't compromised is so much easier.So does Starfleet medical who can't examine infiltrators and determine their species. Maybe Romulans and Vulcans are similar but Rizzo?

And before anyone starts with "why would the head of Starfleet knowingly allow a spy to run Starfleet Security" I'll point out that just a century earlier Starfleet chose a known enemy spy to become the director of their black ops division.I will say I really hope the Vulcan Commodore DOESN'T end up being the HEAD of Star Fleet security because while I could buy that ALL the various Star Empires manage to get spies into each others military organizations, Star Fleet (and Section 31) would have to be pretty damn incompotent to not realize their own HEAD of Starfleet Security is a Romulan agent or collaborator.

Tyler was scanned by a shipboard medical computer three times before Culber found scarring that warranted further investigation. You seriously telling me that a shipboard medical computer aboard one of Starfleet's most advanced ships can't detect a human patient doesn't have proper heart rate or body temperature three times?^^^

This. I've been thinking along very similar lines. They deliberately state that the Zhat Vash is a hatred that goes back thousands of years. That would put it at the same time as the Vulcan/Romulan schism and the rise of Surak. Maybe those early Vulcans learned a valuable lesson about synthetics and AIs that they prefer not to share with off-worlders.

Yes, IF they have knowledge of how to fool medical scans. Look "Trouble With Tribbles was a comedy episode - but in other 'Spy' type episodes, said spies WERE able to trick/fool equipment on all sides because they had been prepared.
