#5 Guardian of the landing party’s property.

Crewman Farrell?
He was Kirk's bodyguard in Mirror, Mirror. He also was in charge of Chekov's punishment in the agonizer booth, and Kirk ordered him to let Chekov out of the booth and to his quarters.
He was played by Pete Kellett who also played a Morg guard and a Klingon crewman(hence the brain reference, and he didn't speak neither episode of Spock's Brain and the Day of the Dove.) As a Morg guard, he protected the Eymorg women; and collected the landing party's hardware. Etc.

Yup. Henoch earns the Starfleet Medal for Gallantry. Pete Kellet was the mystery dude.

#1 , #2 and #3 He (and Fred Carson) were on the surface of Sigma Draconis 6 as Morgs and then they were both down below as 2 hulking bodyguards.

#4 He was in mirror universe

#5 He stopped Kirk from retrieving phasers, communicators and tricorders

#6 Kirk drop kicks him

Etc.

Henoch is now up to bat. Whew!!!

The Medusan Kollos from "Is there in truth no beauty". He is an expert in navigation and is able to help them get back on course after being driven out of the gallaxy by the mad Lawrence Marvick.

Not Daystrom; he caused the problem and did nothing to solve it.

Not Kollos, but this is on the right track.

2nd Clue: Kirk gives a cold shoulder.
 
Kirk seems fairly friendly to Larry. Not Marvick.

3rd Clue: Nice going away present.
 
I'm not feeling good about this answer, but I'll say Matt Decker. He is an expert commander that is on board. He basically helps even though he also places them in jeopardy. Then Kirk has him removed from command (cold shoulder) and then his going away present is sacrificing his life and showing them the way to kill the Doomsday Machine.

Note that he does not look happy about my choice either.

^ That sounds logical. The ‘going away’ could mean Matt Decker being disintegrated, both literally and figuratively.
