Most frightening Star Trek Episode

Scariest episode of all the Star Treks is the 3rd season episode of Enterprise where the Earth was actually destroyed and Archer couldn't remember anything. he lived with T'pol on a hidden planet somewhere with the only Survivors of the Earth that were known to exist. I cannot recall the name of the episode but it clearly was scary watching the Earth get blown up and seeing the survivors tagging along behind the Enterprise NX-01 which had a damaged engine so it couldn't go past Warp 2 or something. It was sort of like a variation of Battlestar Galactica. At the end it was by a less than 1% chance that everything was taken care of. What was the name of the episode ?

I remember being freaked out a little when I was a wee sprog by "The Lights of Zetar", esp the Memory Alpha color-changing and gargling chick and the weird voice of the Lights.
Ditto that! Granted, The Lights Of Zetar wasn't exactly the best TOS episode ever (or even third-season TOS episode ever ), but no one can deny the fact that the Zetarians were genuinely creepy!

(BTW, much of the music used in that episode was originally used in Where No Man Has Gone Before. That probably is a major factor in why The Lights Of Zetar manages to achieve such a creepy effect.)

Those things in Operation Annhiliate used to terrify me as a child.

I thought the pancake brain cells were scary as hell when I was young too. It wasn't until I saw the blooper reel where one hits Spock in the ass that I really got over my fear of them.
