Having two people with the same first name in a show would be a little confusing for today's audience and we already have Airiam who would share that first name with her. Early on he is identified as a Lieutenant Commander while wearing Commander's stripes. I figure the promotion paperwork just got lost in the galactic mail.
My assumption was that he was granted a brevet promotion to Commander, and was allowed to wear the full stripes, while he was only a Lieutenant Commander formally. Final promotion came later.

Actually, Walter's name is one of Stargate's more convoluted stories. In the first four seasons he wore a nametag that said Davis. Then in the fourth season episode 2010, RDA ad-libbed the name Walter. So it would seem at this point his name is Walter Davis. Then in the fifth season he began wearing a nametag with Norman Davis on it, and would continue to do so for the sixth and seventh seasons as well. Then in season 8 the writers finally decided to establish a name for him. They decided to stick with Walter as the first name since that was spoken in dialogue but had to come up with a new last name since there were legal issues with the name Walter Davis. For a last name they chose Harriman, based on a number of fans mistakenly believing that was his name claiming General Hammond called him that in the first season (Hammond in fact called him "Airman") and so in season 8 the character's name is firmly established as Walter Harriman and his nametag is changed accordingly.Same thing happened with Walter in SG-1, except that was Richard Dean Anderson adlibbing.
