Yes, I can't see them necessarily picking a winner and then asking for the composition to be changed drastically - that would be kind of pointless. I think they need to expect that fan submissions might not be quite as polished - or the artist may not have quite as extensive resources as a professional would - and certain allowances may be made that they wouldn't do for professional, commissioned works. That's not to say the fan submissions won't include works that are every bit as good as anything we've seen published in the past - I know I've seen several works right here that surpass most of the pieces I've seen in the calendars.
As for prior publication, I think that refers to actual publication, as in another calendar, a book, accompanying an article online - somewhere the artist was compensated for its use. I don't think that includes something like posting it on your own website or gallery, or in a thread here or another fan site. Of course, I wouldn't recommend posting the full-size image, regardless, but maybe a reduced-resolution, scaled-down image that conveys the concept without providing the full quality of the final calendar print - something where they wouldn't lose money on the calendar because the pristine images were available online already.
