I've never heard of it. Do you first toast the bread and then put egg salad on it or do you toast it with the egg salad already spread on it?

Yeah, you toast the bread first and then you spread the egg salad over the top of the toast. Just like peanut butter and jelly. Simple. Quick. Delicious.
