That wasn't future-John, that was Kyle Reese. He appeared in the hallucinations that Sarah was having after being shot in last seasons finale.

John jumped forward in time which meant that - from a temporal standpoint - there would be no John Connor from the point in time in which he left.I'm assuming that Derek not knowing who Connor was merely meant that this is before Derek heard of Connor, in the earlier years of the war. After all Connor didn't become the messiah overnight. At first he was just another grunt who slowly rose through the ranks. I actually just finished reaching the movie prequel novel (From the Ashes by Timothy Zahn) and in that Connor is just a leader of a small ragtag group of rebels trying to get himself in with the main Resistance.
Honestly, I don't know how much I like this episode as a series finale at first blush, as it definitely seems like the writers and producers threw caution to the wind and completely changed course with the jump into the future. The premise of the show, to me, had always been fighting in the present to save the future. Too much of a sudden cliffhanger to work as a swan song for me.
