Clue 2: The wrong is caused by a love triangle, of sorts.
 
OK, my answer is a love triangle, of sorts, is "The Companion", Cochrane and Commissioner Hedford. Hence, "Metamorphosis" is the episode.
 
"The Return of the Archons"

O'Neil is never shown in uniform and is only wearing the disguise clothes on the planet, which turned out to not be a good disguise.

He is seen wearing a red shirt in "The Tholian Web".

But he does not die, thus creating uncertainty in his red-shirt status.

Where No Man Has Gone Before?
Gary Mitchell and Lee Kelso both wore "salmon" shirts, and both died like true red shirts. :ack: Scotty (engineering department) also wore the salmon shirt.
 
How about "Bread and Circuses"? The "blessings of the sun" is invoked. Not sure of who the red shirt is, but a couple of guys sacrifice their lives without a traditional red shirt uniform on.
 
Balance of Terror?

Tomlinson fits the role of redshirt in this episode since he is the only casualty. However he does not wear red.

Kirk officiates the wedding at the beginning, essentially giving his blessing to the marriage.
