How about "The Man Trap"
1. Not a lot of technology is used, other than heat sensors to detect a body on the planet. Certainly sensors could have distinguished the Man-Trap from a human. Oh, and let's not forget McCoy visually checking tonsils rather than relying on his medical equipment. Strange that he never did that again after this early episode, but maybe this encounter with the salt monster and the loss of Nancy was something he did not want to be reminded of.
2. Kirk-Fu is useless when the Man-Trap paralyzes you.
3. Kirk thought they were doing a routine medical examination and meeting McCoy's old girlfriend. He did not expect a salt-monster.

Journey To Babel?
1) Better technology could be used to locate the alien infiltrator. Also, Kirk’s wound could be healed more efficiently with medical technology.
2) Kirk-Fu fails - he gets stabbed.
3) Kirk thought he was just ferrying passengers, but complications arise.

For Clue 1, I was referring to use of the shuttlecraft to ferry the Vulcan delegation up to the ship versus simply using the transporter. (Maybe they wanted to by-pass the transporter sensors as they smuggled spy equipment on board...probably how the faux-Andorian got on board along with the transmitter.) You correctly got the other clues. At the start of the episode, Kirk says, "The Vulcans are the last delegates we have to pick up. As soon as we get them aboard, we'll be able to relax." How many times have we heard that?
Also, the word "journey" is in the clue.

2. The Enterprise is only seen at the end of the episode as it warps away from the nova sun.
3. We know the ship is "out there" someplace, but it is not a factor in resolving the mystery on the planet nor help Kirk save Spock and McCoy.

I can't help saying "The Empath", based on the last clue. The other clues seem to fit too.
1. Sulu comes in later (maybe 1/3 of the way) in the episode.
2. The Enterprise goes away in the beginning and then comes back at the end.
3. If this refers to the Enterprise, then since it is not featured much in the episode, the teaser probably does not feature it at all.
4. Obviously Gem has very few ... um ... no lines.

2. The Enterprise is seen very briefly more than halfway through the episode.
3. Given 2 above, the trailer probably does not have the Big E in it.
4. Someone has to have the fewest lines and someone has to have the most. (I suspect there is more to this clue than I can figure out)
5. There is unique music here. I'm not so good at remembering what other episode it is in, but I'll gamble that it is not often played. (note that my willingness to gamble is inversely proportional to the amount of money i can lose in the game)

I'll guess "The Gamesters of Triskelion".
I can make some of the clues fit, but mostly I pick it because it is one of the episodes I have not eliminated based on clues and previous guesses.
1. Not sure about this, but three brains finally show up at the end.
2. Spock, crew and Enterprise finally catch up at the end.
3. Didn't have time to check the trailer, but it might be true.
4. One of the Thralls does not speak.
5. Not sure about the music and I didn't want to go back and listen to the whole score.
6. The only deaths are a couple of Thralls
