Yep, good catch! It's the Haakskeekah stone made from a "lesser crystal" used by the Skekses in the "Trial by Stone" ceremony.
As mentioned above, Tarkin suspects Vader is Anakin but never had it confirmed. I doubt Yularen knows or even suspects. It might be interesting to know Vader's current opinion of Yularen, given the two of them worked together in the Clone Wars. For that matter, I wonder if well ever see a reunion between Vader, Yularen and Rex? Sure would be awkward.
I don't think Vader's identity as Anakin Skywalker is a secret as for most people it's just never brought up. Vader really does consider himself a separate person than Anakin, and could very well have never admitted to anyone he was Anakin Skywalker until revealing the fact to Luke.
Except Piett replaced Ozzel who must had some say in running the fleet given his ill-fated decision on when to bring it out of hyperspace.

Actually it was a closely guarded secret. Palpatine even went so far as to purge any medical or genetic records of Anakin, lest he be identified, or worse: cloned.
And yeah, in a sense Vader did consider himself a separate person, though not in a delusional sense. It's more akin to how one might think of oneself back when they were very young children. Perhaps a more apt comparison might be how a particularly self-righteous born-again might think of the version of their past self before they "saw the light".
Technically and undeniably still the same person, while at the same time profoundly and in almost every way that matters: not.
Yeah, I don't think it's a mystery that Ozzel happened to have a Captain stood next to him on the bridge of the Executor. Obviously Piett was the ship's Captain.
By RotJ one would assume that role was filled by Gherant. The actor playing him was indeed credited as "Stardestroyer Captain #1" and he's referred to as "First Captain" in the novelisation, though I think in the script he's just listed as "Commander".
Take it easy, @Reverend . It is understood that you disagree with @Christopher , but it's not necessary to go to these lengths.
It's less of a particular disagreement than weariness at a pattern of behaviour that persistently derails discussions. But you're right, that was a tad unnecessary.
It certainly sounds like Genevieve O'Reilly... I'm trying to confirm it now with a link. It would certainly go hand in hand with Forest Whitaker reprising his role of Saw Gerrera last month.

They've gotten other original cast members to lend their voices to their roles in the past - do we know if Ewan McGregor is going to be in this?
No, they've already confirmed who is voicing Obi-Wan, it is Stephen Stanton.
He voices Tarkin in the show, he also voiced Admiral Raddus in Rogue One.
I've never been that impressed with Stanton's Tarkin. He doesn't do nearly as good an impression of Peter Cushing's voice as Guy Henry from Rogue One did. (I was going to say "the guy from Rogue One," but I figured I should look up his name rather than just call him "guy," and it turns out his name is Guy!)
I've found his Tarkin much better in Rebels than it was on Clone Wars.
My two main take aways from that are: -
1) Yay Dantooine! (fingers crossed for Jedi enclave ruins!)
2) Dammit, I've been mispronouncing Chandrila in my head for over 20 years! (but then, so have the Random House audiobook people =P )
Side note: I'm about 80% sure that is indeed Genevieve O'Reilly playing Mon. I quickly threw on 'Senate Murders' and the Rogue One teaser to compare and this performance certainly sounds a lot more like O'Reilly's that Soucie's. A possible giveaway is that her voice has that slightly flat tonality you sometimes get when casting actor more used to screen work than voice work and they haven't quite managed to unlearn not to play it too big for the camera.
Anyone else spot the curious omission in that scene? Hera introduced her crew as just Zeb and Ezra. Where's Kanan? Chasing Maul perhaps?
I've always thought he was perfectly convincing as Tarkin. But then, I try and judge such things based on how well they capture the character, not just how well the impersonate the actor that originated the role. There's a subtle but important difference there I think.

I expect it probably was. I just saw the CW Tarkin episodes this morning, and he was pretty far off base there, but I don't remember him being quite that far off on Rebels.
Then again, some names have more than one pronunciation in canonical productions. On TCW, they tended to pronounce "Twi'lek" like "Twee-lek," but I think on Rebels they say "Twy-lek" instead.
Well, I tend to have a very precise memory for sound patterns like speech rhythms, so in general it takes a really, really accurate impression to satisfy me. Also, more specifically, Peter Cushing just had such a terrific voice and performance style. It's not just about the accuracy of the impersonation, it's about how good the acting is, and Cushing's hard to match on that count.
It is so far, though there's yet still time for at least one of them to show up.
