I remember the Carrie jump moment very well. It's still creepy.
I tend not to get too jumpy with film and TV shows, but occasionally they get me. The Ving Rhames version of Dawn of the Dead had a great jump moment when we see a rather messy looking zombie suddenly appear up against a window.
The 1991 mystery movie Dead Again starring Emma Thompson and Kenneth Branagh had a really bad one. The soundtrack goes utterly silent and then Thompson screams something like "SOMEBODY HELP ME!" at the top of her lungs. I happened to be watching this late at night and I thought it would wake the whole house up!
Doctor Who has actually provided TV's only jump moments for me in recent memory. A couple of appearances of the Weeping Angels in Blink did it to me, and there is also a great jump moment in the just-broadcast Waters of Mars that I won't spoil since it hasn't aired in the US yet.
