Wesley would be the older Step-Son who came back from his Sabatical Treking around with "The Traveler" and continue his career in StarFleet.

In when Picard was in the nexus, he had kids. His oldest child was name after his nephew Rene. Picard ask Guinan, were they his kids and she said yes.
If Picard gotten marry between lost of the Enterprise D and when gotten command of the Enterprise E. All his Kids would all be serving somewhere in The Federation. Either on Starship, starbase, at Starfleet Academy or somewhere else.

Not necessarily. Some might have gone their own way like Jake Sisko did, not joining Starfleet. Picard had lots of random interests that could've inspired his kids to do other things. An archaeologist, an equestrian, master tinwhistle player, for example.
