For some reason now I'm picturing the credits for the show after Janeway goes nuts passing out promotions:That's a variation on the "ship full of captains and admirals" argument. It would have taken literally hundreds of promotions to get to that point. Surely there was a reasonable medium between that and the two promotions Janeway actually gave. Especially given that she lost 39 of her people to death, and demoted six more.

It's subtle, but every time Janeway sees that she is not home, that this time line is some where absolutely alien, and this is not her real TimeLine where loyal Mark is still waiting for her... F*** you Harry.

^Timeless? I can't recall a promotion being seen or mentioned in that episode, only that he left Starfleet in frustration.
(I would expect Starfleet to at least give him a promotion when he returned after 7 years, but I can't assume it without proof).
Postponing is definitely possible. Look at the Doctor... he asked for a name in S1, and he never got one. Schmullus, Van Gogh, Schweitzer, Dr. Zimmerman, and Kenneth were all very legitimate possibilities, but none were used.
And you're right about the "young Ensign Kim" nonsense, even though by series end Harry is as old as Riker was when he was OFFERED A COMMAND. But they focus heavily on Harry's green-ness and ignorance.
As an example, look at the Wiki, it states that he "doesn't know how to address his captain". Not true: he addressed Janeway as per Starfleet protocol: "Thank you, sir." The fact that Janeway prefers "captain" (or "ma'am" in a crunch) is simply one of her her idiosyncrasies, which a new crew member can't be expected to know. But since they want to cast Harry in a certain light, that's how they call it.
