well, the rules of the 24th century have been shown to be different. Subordinates and supervisors in Starfleet apparently have romantic relationships, so why not for doctor/patient?. I can't remember offhand, maybe I'm remembering wrong, didn't Dr. Crusher get involved in a relationship with a patient or two?
