Actually, nice American restaurants (obviously not normal chain restaurants) are the only places I've seen bread for free. I've seen bread included in the meal, but there's an automatic cover charge for it. In America, you could theoretically eat bread and drink water and not pay a cent (although they'd probably kick you out
