What if JD hadn't gotten scared in My Fault?

In the Scrubs episode My Fault, JD and Elliot finally got together but by the end of the episode, JD suddenly thinks "what do the hell am I doing?" and gets scared which leads to him breaking things off with Elliot in the next episode. But what if JD hadn't gotten so scared and continued being together with Elliot? Would anything have changed?

Well, he obviously wouldn't have hooked up with Kim and had the kid. Glad he finally grew up to be honest.

Yeah, Sam wouldn't have been born and Izzy would have been without a husband. Why do you hate Sam and Izzy?

^ Given how clucky Elliot is it more than likely JD and her would have had a baby and it might have been a "Sam".
