The 47 appearances started in TNG, and by Joe Menosky. He graduated from a college that had some club that said 47, or an iteration of it, was everywhere. Joe Menosky later ended up writing since season 2 of VOYAGER, ending up as Co-Executive Producer in season 6, which was his last season.
For the other half of the question, I actually have no idea. I never realized someone had started "some kind of" phrases as an in-joke.

I think it was Anthony Crivello, in the episode "WARLORD". Can't remember the character's name, though.

OK. A question for you or for anyone else who knows the answer: An African-American actor plays a character in an early season of VOY. Due to the character's nature, you can't make out the actor's face. Then in a later season he returns and this time we can see his face.
