Don't know the canon or litverse answer, but in my mind the colonies started by Earth pre-federation were not automatically put in the federation and had a choice when the initial creation happened. If they chose not to then they had to go through the normal process later on if they changed their mind. The ones that were lost due to bad record keeping of loss of communication and found later, I'd say those had to go through the normal, even if fast tracked, application process.
The Enterprise-D must have had other counsellors. A ship of that size at least 2 per shift would be reasonable.
I've thought about this too, it's 1000 people, that means you could theoretically see Deana once every three years. I'm sure tons of people needed therapy after some of the traumatic stuff they go through like Best of Both Worlds.
It's probably closer to 800 actual crew aboard. Presuming she might only have to see many of them for annual health assessments, because she's also apparently instrumental in crew evaluations (Per Lower Decks) then she can likely schedule those at like 14-16 per week, leaving the rest of her full time hours to focus on the regulars, that need therapy.
We can also perhaps figure that 24th century therapy is more advanced & requires less investment. For a heavy influx, after a disaster or something, they can put into space dock for more help, or there's also others qualified in psychology for even Troi herself to speak with (Per The Loss)
I'd think it's doable
There are possibly civilian long-term passengers she never sees, because they never seek out or are referred to counselling. While Starfleet might require a physical screening to keep communicable diseases from spreading shipboard, they don't necessarily demand that a five year old child or a waiter at 10 Forward see Troi before they can move into their quarters.
Like Ben in Lower Decks. This guy has no obligation (nor need, by the look of him) to be getting counseling lol. I imagine most people, even crew, wouldn't. She'd just do routine evals on the crew, like yearly, & help people with issues
Also worth noting that Troi is always referred to as "ship's counselor." Not "head counselor" or "chief counselor" or anything that would imply there are others. Crusher is consistently referred to as "chief medical officer" and we frequently see that there are other doctors on board, however.
BTW, as someone who still watches broadcasts of TNG (on Pluto TV nowadays) imho the #1 most disruptive aspect to its general rewatchable continuity is Troi's outfits. I can see the varied crew, like Yar, Pulaski, Ro or Barclay, & it doesn't take me out of the show.
The various style & production design differences between seasons is noticable but not wholly jarring. Even Wesley growing up isn't terribly disorienting. Nope, the one thing that really dates the episodes is wtf Troi is wearing & how her hair is done lol. It's just so... all over the place

So, I saw part of TNG's "The Host" yesterday and I got to wondering: why is there cold smoke coming out of the side of the shuttle? What is the purpose of that? How come we don't see that all the time? How comes the shuttle needs like half an hour to be prepped, but somebody (other episodes) can sneak in and fly out with no half hour prep.
Deanna just tends to the main cast, and the other interesting guest cast member.

Well...we have seen doctors do what basically amounts to counseling from time to time. Maybe they are all trained to do it on a basic level and only the "servere cases" do get sent to Troi?
Oh I don´t know...her first few outfits are consistent by all having that "enter here" arrow pointing to her vagina...just different colors. Not so much, because there's continuity in how dorky he always was
There'd maybe be more aesthetic consistency if he would've never been there at all, but when he is, it's always just "Oh, here's the goofy kid" didn't matter what he was wearing IMHO
Troi throws off the consistency more, because she's almost always there & in season one, those awful gray suits, that updo & her acting deliberately poised are especially distinctive.
Then she softened it with the maroon jumpsuit & relaxed hair look, which they probably should've just stuck with, but for a while she starts wearing that slinky teal slip of a dress on the bridge & mauve jumpsuits. It was kind of all over the place
Finally, they put her back in uniform, thankfully not the one from the pilot, that she has to squeeze back into for the finale lol, but that look (which she should've had from the get go) gives those later episodes their own distinctive look too. Nothing dates any given episode more IMHO than Troi, not even Riker's unbearded face or Worf's haircuts

They may have gone looking for other surviving Gomtuus (Gomtii?).
I had an idea that they might band together with the Farpoint space jellies, the "Galaxy Child" alien, the ST IV whale probe and a few other space-based organisms to impose a quarantine on planet-based organic life. "We don't want you out here fucking up our environments."
