^ Yup, it happened during that narrow hallway-shootout. She walked forward alone, and then got shot.

I think I saw somewhere where they already admitted that was a mistake in the comic.

Nothing that a future novel or comic couldn't fix, really -- she could've been an identical twin or something (since she's only identified as "Lt. Owosekun" in the issue, and no first name is ever provided).
