It's just that Kirk had just called her "Mrs. Sarek" at which point she invited him to call her "Amanda" as he couldn't pronounce the Vulcan name.
It's unfortunately ambiguous which name she's referring to. I'd always assumed it was the Vulcan for "Sarek," but it could be her given name in place of Amanda; she might have changed her whole name upon getting married, not just her family name.
And that's not even getting into the fact that Kirk called her "Mrs. Sarek."
Perhaps the same reason folks call Kira "Nerys",

Though, (most) angry mothers are known for making their point known by using full names.

I think she does call her Nerys in the weird episode where Kira goes back in time and finds out her mom was Dukat’s consort.

IIRC, Kira had "Nerys" on her baseball jersey when everyone else had family names.

"Kira" and "Nerys" in different shots, actually. I guess someone in the costume department didn't know that, while it comes last, Nerys isn't her "last name," and it was only caught and changed once everyone saw the jersey. Leeta's jersey also only said "Leeta," because she never had a family name established (I remember some source suggesting that she'd been a war orphan and it was her choice to have a mononym because she didn't know her family).
Never could keep that straight.

That's really cool, I never heard that. Great idea that adds a lot of depth to the character.
