OK we know it's been a running theme in "new who" and many points about it have been covered, but who actually started the time war and laid the first shots?
In one of the later Seasons of the Galifrey Audio Series from Big Finish, we see (hear) the CIA director "tricking" the fourth Doctor into trying to "stop" the Daleks from being created. It took Narvin about a minute and half to realise that this wasn't the headshot he thought it was, and that the Daleks were going to figure out what he did, and strikeback hard. The writers allude to this being the start of the Time War without using the words Time War.
Depends on who you talk to. The Daleks will go around blaming the Time Lords for sending #4 on a secret mission. But the Time Lords will point out that the only reason they did that is because the Daleks are such ginormous douchenozzles.
I'd say, in their effort to prevent it, the Time Lords themselves actually began that long process.
However, while Genesis is an early indication, Rememberance and Big Finish's Apocalypse Element are by RTD the first firing shots by the Time Lords and Daleks respectively.

More or less, from memory the Time Lords foresaw a time when the Daleks had destroyed all other life in the universe and they had become the dominat life in the universe. So they basically sent The Doctor to Skaro to either avert their creation, affect their genetic development so they eveloved into less aggressive creatures or to discover some inherenent weakness.
