Well it's not certain that 154 is his exact biological age. In the Search for Spock he's regenerated and aged to a point that certainly appears to be his age when he died but that's not really certain, especially for a Vulcan. Tuvok looked the same age for about 90 years.Spock was pretty young for a Vulcan when he died. 154 or so? Maybe five years on this other universe effected his health? Or maybe because he is a hybrid he didn't have as many years as his father did.
I doubt Spock had the photo with him when he came through the black hole.
He probably just downloaded it from the Starfleet archives and kept it on his person while on New Vulcan. Pretty simple.

Good question. We know nothing of how he died. He may not have been in a situation where he could give it to someone. Or, because of his background in the Prime universe and the existence of Spock in his new universe, he thought it best to not give his katra to anyone.What happened to his Katra?
