    The Doomsday Machine, or planet killer.

    1. There is no doubt it exists because the planets are rubble
    2. It's real, but initially described as something else when Decker said "it was right out of hell".
    3. It is very very old, and probably from another galaxy, hence traveling for millennia.

    The Yonada spaceship? It's first considered a fake asteroid, before Spock scans inside and discovers the spaceship.

    Even though it was a reuse of the The Paradise Syndrome asteroid, it is still a special effect, so, nope.
    Ditto, they used a special effect to "materialize" the swords.

    (He fires the phaser at the Salt Monster, who is disintegrated), just as your guess is disintegrated.

    Close on TAN RU, but based on it morphing with NOMAD, it wasn't fully destroyed with some of its programming and parts surviving.

    Enterprise's sensors confirmed its presence.
    Clue 2: This object is real, though its initial appearance was confused as not real to something else.
    Visual sensors showed nothing there. They discussed whether it is a Romulan vessel.
    Clue 3: The object is probably old and possibly millennia old.
    Bele mentions that he was chasing Lokai for 50,000 years. I assume that he used his ship for most of it. Bele said, " It served me long and durably".
    Clue 4: I guess to save money on special effects, they used no special effects.
    Speaks for itself.
    Clue 5: The object was destroyed (unceremoniously in my opinion.)
    Disintegrated in space after teleporting Bele onto the Enterprise. (And with no explosion or fanfare.)
    Clue 6: Based on the object's un-appearance, I am unable to visually describe the object.
    Visually, it was never seen:
    SPOCK: A most unique craft. Pity it couldn't be salvaged for our study.
    BELE: Yes. It was sheathed in special materials that rendered it invisible.

Many are equally good and are as well respected, but wherever we have gone, no words have said this thing of importance in quite this way. Look at these four words written larger than the rest, with a special pride never written before or since. Tall words proudly saying 'Your Guess Is Wrong!'.
