1. Reality is not all there; half buildings, etc.
2. I shot the sheriff?

How about The Deadly Years? He's in command at the start of the episode, then he's relieved by Stocker, then he commands the Sickbay operation (that ep has some great suspense) even though he has no authority, then he shows up on the bridge without technical authority and everyone defers to him (and understandably so) as he saves the ship.
 
Not The Deadly Years nor the The Enemy Within, but you both seem to be on the right track of multiple commands.

Based on your avatar picture, it looks like the dikironium cloud creature has already visited you. I doubt that Kirk feels that the creature is an old friend. Sorry, but not Obsession.

Okay, new clue:

1st Clue: Kirk's in command, then not, then is, but not, then not, but is.

2nd Clue: Kirk searches vast distances for a friend.
3rd Clue: Kirk to the rescue.
 
The Galileo Seven.

1. Kirk's in command at the outset. Then the persnickety - and unevenly acted - Ferris technically relieves him. However, Kirk issues orders ("space normal speed") to Sulu while under Ferris's technical command, then finally persists under that state of affairs to rescue the crew. (By the way, did anyone ever think how much less interesting the rest of Star Trek would have been without Spock, Scotty, and McCoy?)

2. Kirk commissions a painstaking search of the planet.

3. Kirk sees the flare and orders the crew to "activate beams."
