From what I understand, Sulu's first name was given in the novel 'Entropy Effect' (someone correct me if I'm wrong)

Correct. Coined by Vonda McIntyre, and suggested to director Nick Meyer by novelist Peter David, during his ST VI set visit, that the name be slipped in as an Easter egg.
