How did they meet? He helped rescue her from the Kazon but was he waiting for her when she came up from the subterranean Ocampa civilization originally? Was it a chance that he was just there when she came up the first time or did they originally meet somewhere else?

I suspect that when he was in the camp last (when he "borrowed" their water) he met her then. There is nothing to say how long she was on the surface on when his last trip to the planet was. He may have felt sorry for her as a slave and taken some time to get to know her and looked for ways to rescue her.

Neelix met Kes when he was trading with the Kazon-Ogla. She was a prisoner in their camp. Neelix fell in love with her and planned to rescue her from the Kazon. he stole water and gave to her but the kazon did discover that and he had to run away from the planet. He was planning something to rescue her when he encountered Voyager and you know the rest.
