The thing with Q is all his gestures are meaningless because they take no effort and cost him nothing. The one thing that apparently would pain him is to act with no self-interest and he seems to have never figured out that even attempting such would get him human brownie points. Neither Picard or Janeway is going to bed him no matter how much worthless wooing he does because 99% of it is just him enjoying the sound of his own voice.

(I don't believe people can act with literally zero self-interest, but it's possible to at least value others above yourself.)
